No Therapist-Client Relationship

This notice describes how protected health information about you may be used and disclosed by Hayley Hoffman Psychotherapy LLC, how you can access that information, and what rights you have under federal law. Marketing and the sale of protected health information also generally require written authorization, and this practice does not sell your protected health information.

Provide treatment, including coordinating or consulting with other health care professionals involved in your care when clinically appropriate.
Bill and collect payment for services, determine insurance coverage, or respond to payer requirements.
Run the practice, support compliance, improve care, conduct quality-related activities, and manage business operations.

Mental Health Information

This is a psychotherapy practice. Mental health records may be subject to additional protections under applicable federal or District of Columbia law in some circumstances, and we will apply those protections where required. The HHS model instructions specifically contemplate adding language where mental health records may be subject to additional limits.

Our Responsibilities

We are required by law to maintain the privacy and security of your protected health information, provide you with this notice, and follow the terms of the notice currently in effect. If a breach occurs that may have compromised the privacy or security of your unsecured protected health information, we will notify you as required by law.

We may revise this notice, and any revised notice may apply to all information we maintain. Any updated notice will be posted on our website and made available upon request. HHS states providers may revise their notices and must make updated notices available, including on their websites when they maintain one.
